FSA GENERAL REFERENCE

  Read form for specific instructions on deadlines and how to submit to FSA. 
  Forms not submitted timely can NOT be considered.
  Coaches: The purpose of this form is to inform FSA of scheduling conflict in advance (categories listed on form) you team may have for the upcoming season.  We will try our best to accomodate requests, but there are no guarantees.

GAME DAY FORMS

• FSA Game Day Roster / Misconduct Report
  This is a generic blank form.  Complete this form with coaches and players names and jersey numbers.  Give to referee prior to the each game.  Prefilled forms are available for FSA league play, by logging into www.sportspilot.com.
• Sit Out Verification Form (FSA)
  Form must be presented to the referee and signed to receive 'credit' for a sit out.  Fax form to FSA at 972-712-3511.
