The Academy form is specific to an individual player and  should not be completed or paid for by the Academy team.

This form is required for player participation in all NTSSA academy programs or tournaments.  

• Current FSA recreational players are not required to register and pay an additional fee for Academy.  Please email contactus@friscosoccer.org  to request an Academy form for your player.
   
• Recreational players who also play on an academy soccer team, MUST have a validated/approved academy form from their recreational home association.
   
• Academy ONLY registered players can register with FSA no matter where they live.
   
• Registered Academy Players are not placed on a team by FSA. You must find your own academy team.

NO REFUNDS FOR ACADEMY REGISTRATION ONCE THE FORM IS SIGNED BY FSA.

FORMS SUBMITTED AFTER 12PM ARE NOT GUARANTEED TO BE RETURNED THE SAME DAY.

We will provide you with an APPROVED/VALIDATED academy form, but you can also find it in your GotSport Account in the FORMS MENU under PAST FORMS. This form becomes your proof of registration.

Academy Managers - please see the document below for specific instructions for the requirements for obtaining validations for Academy Tournament Rosters as well as specific instructions regarding where and how Academy Players should register as an Academy Player.  

***Academy registration for Fall23/Spring24 is only for players born between 2014 to 2017.  Please follow the steps below to submit and complete your registration.

Step 1.  Register as an Academy Player (Fall2023/Spring 2024) by clicking the link.
**Only valid for U7(2017)-U10(2014) aged players.

Only a parent or guardian should register a player, not managers or coaches.

Registration Fee: $25 for Full Soccer Year (Fall and Spring (8/1/23 - 7/31/24))

Step 2. After registering, you will need to request an approved form for your player by  FORWARDING your registration confirmation email to FSA at contactus@friscosoccer.org.

Step 3.  Please include a copy of your player's birth certificate in the email request if this is your first time with registering with FSA of if you were unable to upload it during registration to the office at contactus@friscosoccer.org for the approval.  The form will be returned following validation.
